Yelena Chorny

Dr. Yelena Chorny is a leader in the creation and delivery of compassionate, evidence-based care to people with substance use and concurrent disorders. In both her clinical and leadership roles, she strives to activate people’s potential and drive personal and system-level transformation.  

Dr. Chorny currently works as Chief of Addiction & Trauma Services at Homewood Health Centre. She has been instrumental in developing innovative services, including Homewood Health Centre’s Guardians Program and Women’s Trauma & Concurrent Program, Guelph’s Rapid Access Addiction Medicine Clinic, and the Cat Lake First Nation Wellness Program.  

She is certified in both Family Medicine and Addiction Medicine by the College of Family Physicians of Canada and graduated in 2024 as a Bregman Scholar from the Rotman Global Executive MBA program for Healthcare and Life Sciences.  

She lives in Guelph, ON, with her family.
